AdventHealth Medical Group is looking for an experienced APP (APRN or PA) to oversee the West Volusia County Transitional Care Clinics.
We are seeking an APP, with a team player mentality, and a dedication to continuing our tradition of excellence in both clinical practice and professionalism, while maintaining AdventHealth’s mission, vision, and values.
The position will require the ability to manage complex, high risk, high frequency patients with the goal of reducing inpatient readmissions. The APP will collaborate with an on-site pharmacist, respiratory therapist, social worker (community care), and diabetic educator, as needed.
The opportunity has a 4- (10) Hour weekday schedule, with clinic locations in close proximity to our AdventHealth Deland and AdventHealth Fish Memorial campuses.

The practice is a part of AdventHealth Medical Group. AHMG is a sister organization of AdventHealth which employs 350+ physicians in 27 individual service lines. In 1996, the Adventist Health System purchased Volusia Medical Center and in 1998, renamed the facility AdventHealth Fish Memorial to honor the hospital’s benefactor, Judge Bert Fish.
