Join Our Team: Palliative Care APC Needed in Defiance, OH!

Join our compassionate team dedicated to providing exceptional palliative care services. As an APC specializing in palliative care, you will manage a patient panel and provide consults to support individuals with terminal illnesses. This role offers the opportunity to deliver high-quality, patient-focused care in a supportive environment.

 

Practice Support: 

  • Certified Medical Assistants (CMA's), Licensed Practical Nurses (LPN's), Registered Nurses (RN's)
  • Practice Manager
  • Other Ancillary Services: Licensed Clinical Social Worker (LCSW), On-site Laboratory and Radiology Services, Certified Diabetes Educator (CDE), Outpatient Surgical Services
  • 23-bed Inpatient Unit on the same campus

Schedule:

  • Monday to Friday, 8:00am - 4:30pm

 

Qualifications:

  • Current APC licensure in Ohio
  • Board certification in Palliative Care preferred
  • Strong commitment to patient care and interdisciplinary collaboration
