Job Description
SSM Health is seeking a compassionate and dedicated Nurse Practitioner or Physician Assistant to join our healthcare team in St. Charles County, Missouri. The FNP will provide high-quality primary care services to patients of all ages, emphasizing health promotion, disease prevention, and patient education. The ideal candidate will be committed to delivering patient-centered care and improving the overall health of the community.
Key Responsibilities:
- Conduct comprehensive health assessments, including patient histories, physical exams, and diagnostic tests
- Develop and implement individualized care plans based on patient assessments
- Diagnose and manage acute and chronic illnesses
- Prescribe medications and other treatments in accordance with state regulations and practice guidelines
- Provide preventive care, including routine check-ups, immunizations, and health education
- Collaborate with physicians, specialists, and other healthcare professionals to ensure continuity of care
- Educate patients and families on managing medical conditions and promoting healthy lifestyles
- Maintain accurate and up-to-date patient records in the electronic health record (EHR) system
- Participate in quality improvement initiatives and professional development activities
Qualifications:
- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of Missouri
- Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP)
- Minimum of 2 years of experience in a primary care setting preferred
- Excellent clinical, diagnostic, and therapeutic skills
- Strong communication and interpersonal skills
- Ability to work collaboratively in a team-oriented environment
- Proficiency with electronic health records (EHR) systems
Work Environment:
- Full-time position with regular office hours
- May require occasional evening or weekend shifts based on patient needs
