Adirondack Medical Practice is a privately owned Primary Care Practice group actively seeking a friendly and motivated Nurse Practitioner and/or Physician Assistant to join our team. Responsible for conducting patient visits/assessments related to acute and chronic care management. Assist with the development, implementation, management, and achievement of specific programs / objectives and core initiatives. These would include coding proficiency and documentation/completion of clinical objectives required for our status as a Patient Centered Medical Home.

Licensing Requirements:

• New York State Nurse Practitioner or Physician Assistant license.
• Must be in good standing with New York State with no restrictions to practice.
