Job Description

Seeking a full-time nurse practitioner to join Providence ElderPlace's Program of All-Inclusive Care for the Elderly (PACE) in Portland, Oregon. This program provides comprehensive care, social services, psychiatric care and acute care to geriatric patients across the Portland Metro area. As a member of this team, you will be providing these services in patients homes as well as in medical clinics. You will also work closely with LCSW Mental Health Case Managers while helping to integrate behavioral health services into the program as well. This is a great opportunity to be part of a highly compassionate group of professionals serving some of the most vulnerable members of the Portland community.

  • Monday to Friday, daytime hours
  • Outpatient primary care setting including home visits
  • Collaborate with physicians, nurse practitioners, nurse care manages, social workers, pharmacists, mental health case managers, and other team members
  • Must be a board-certified/board-eligible Psychiatric Mental Health Nurse Practitioner
  • Ideal candidates will have an interest in obtaining MAT Waiver for prescribing Suboxone

Providence ElderPlace is a Program of All-inclusive Care for the Elderly (PACE), and is the first program of its kind in Oregon. With eight conveniently located centers in the Portland area and on the Oregon Coast, Providence ElderPlace provides and coordinates all necessary care services for seniors to remain active within their communities. The PACE model offers medical care, physical and occupational therapy, lab tests and procedures, home care services and more.
