Memorial Hermann Memorial City Medical Center is seeking a licensed Advanced Practice Provider to join a highly successful primary care solo practice in West Houston. 

• Outpatient Practice
• Monday-Fri 8am-5pm 
• Call Coverage - Weeknights only, Phone Only
• Patient  Volume | 15-20 pts. per day
• Basic Procedures Including Well-Woman Exams Performed In-Office
• Telehealth visits available
• EMR system - E-Clinical Health, experience preferred
• Training available 
• Well-established Patient and Referral Base With a Diverse Payor Mix
