Job Description

SSM Health is looking for a part-time or full-time advanced practitioner to join our innovative direct-to-employer program at the Grande Cheese clinics in Wyocena and Friendship. This unique opportunity focuses on providing a patient-centered approach to both primary and acute care for Grande Cheese associates and their families.

Employment Type: Part-time and Full-time flexibility

Locations: Wyocena and Friendship Clinics

Patient Population:

  • Primary Care: Ages 5 and up
  • Acute Care: Ages 2 and up

Weekly Schedule Example:

  • Monday: Wyocena Clinic (7:00 AM - 4:00 PM)
  • Tuesday: Friendship Clinic (7:30 AM - 2:00 PM)
  • Wednesday: Wyocena Clinic (8:00 AM - 5:00 PM)
  • Thursday:
    • Wyocena Clinic (8:00 AM - 11:00 AM)
    • Friendship Clinic (12:15 PM - 4:30 PM)

Key Responsibilities:

  • Provide independent patient care
  • Conduct comprehensive primary and acute care visits
  • Perform pre-employment and work-related physicals
  • Develop and participate in population health programs
  • Utilize EPIC Electronic Medical Record system
