Highlights
This provider will join our Touchpoint HATS (Healthy Aging Transition Services) practice providing outpatient consultative care to the geriatric population at our clinic on the campus of Community Hospital East. Ideal candidates will have demonstrated expertise in the advanced practice of nursing within the Adult/Geriatric population and an understanding of the specific needs of this population.
Touchpoint HATS is an outpatient clinic for comprehensive assessment of common issues related to aging, such as memory problems or falls. Our team includes a geriatrician, a social worker and a nurse practitioner. Patients may be self-referred, referred by a loved one or referred by a physician. HATS provides a wide range of assessments and services, including the following:
