MedZed Physician Services utilizes telemedicine technology to bring healthcare to patients with complex medical, behavioral, and social needs. Our practice includes Primary Care Providers, Behavioral Health Specialists, Palliative Care Team, nurses in the home and care coordinators. Our nurses visit patients in their homes and back to the clinicians over our novel telemedicine platform. This allows us to visit many patients in their homes without having to drive. We partner with health plans and managed care organizations to care for high-risk patients to help them stay healthy and out of the ER.
We are currently recruiting compassionate Primary Care Providers to treat patients remotely over our telemedicine platform in a team-based model of care. We provide all infrastructure support, telemedicine training, administrative support, office space, EHR, and patient recruitment. You will manage an adult patient panel, attending to chronic, acute, preventive and sometimes palliative care needs. You will work with a team of nurses in the field who visit your patients, and practice the functions of self-management skills training, and operate the telemedicine platform as you review and interact with patient. Our team takes care of scheduling, coordination of services, records retrieval, rapid patient check-ins between visits and billing.

In 2014, childhood friends Scott Schnell and Neil Solomon, MD, first discussed an emerging frontier in health care — telemedicine. Given their awareness of a rapidly aging population, rising health care costs and a looming shortage of homecare providers in our country, Scott and Neil recognized an opportunity to step in and make a difference. Our members slip through the cracks of the traditional delivery system. They don’t regularly go to the doctor, whether due to lack of transportation, lack of trust, or language and cultural barriers. They may have access to some social services, but those are fragmented and not integrated with their medical needs. Now, more than ever, we need to provide care for them, delivered at home. These members are often hard to find and while health plans spend substantial resources on these members, outreach efforts typically fall short.
