The Confluence Health Anticoagulation Clinics (AC Clinics) are operated by a multi-disciplinary team of pharmacists, nurse practitioners, physician assistants, registered nurses, and medical assistants to provide outpatient anticoagulation care to approximately 4300 patients throughout North Central Washington. Spanning 4-counties there are 9 clinic locations including: Wenatchee, Cashmere, Waterville, Brewster, Omak, Tonasket, Oroville, Moses Lake, and Ephrata, with management services being provided via in-person, telemedicine, and virtual appointments. As an Anticoagulation Center of Excellence, we provide comprehensive anticoagulation services including warfarin, DOACs, injectables and peri-procedural management.

What We Are Seeking in a Candidate
- Nurse Practitioner or Physician Assistant
- Certified Anticoagulation Care Provider (CACP) certification within 2 years
- Ability to obtain an active license in Washington State
- Concise and compassionate communication skills to educate patients and families.
- Ability to thoroughly document and maintain comprehensive patient records.
