CentraCare is hiring an Advanced Practice Provider, able to see ages, to the Diabetic team in Willmar, MN.
Responsibilities
The Advanced Practice Provider (Nurse Practitioner or Physician Assistant) is an expert in the field of diabetes providing direct patient care including assessment, diagnosis, planning and treatment of health problems, health promotion, and preventative care in AADE recognized program for diabetes management. The provider will work independently with patients of all ages to manage diabetes treatment regimens and education to include noninsulin and insulin medication starts and dose adjustments; provide consultation and management recommendations related to diabetes complication risk reduction and evidence-based quality care outcomes. In addition to direct practice, the sub-roles of the APP include: staff/patient/family education, evaluation of patient care outcomes, research, providing professional consultation, and providing interdisciplinary collaboration.
