We are seeking the brightest and most innovative Advanced Practitioners to help us deliver the highest level of heart care to our Advanced Heart Failure (AHF) patients. Come and be a part of one of the largest and most experienced multidisciplinary teams in the nation!

Do you have what is takes?

• Board Certified Physician Assistant or Nurse Practitioner
• Active or ability to obtain active license in Georgia
• Prior Cardiology experience required
• Must be progressive-minded, self-starter who takes initiative while working collaboratively with supervising physician(s) and other clinical staff.

Opportunity Overview:

• Outpatient only
  • Primary location in Gainesville
  • Outreach clinic coverage twice per month
  • No weekends or call required
• Development and implementation of teaching plan with the patient and family
• Collaborate with the patients, patient representatives, and interdisciplinary team to facilitate optimal outcomes as the patients transition through the healthcare continuum.
• Provides patient navigation, ensuring care protocols and quality metrics are adhered to and that patients receive appropriate referrals and follow-up care.
• Works with physicians to establish and maintain optimal workflows, alignment, and standard work to serve the cardiac needs of patients.
• Participates in creation of an innovative model of care delivery while fostering and maintaining relationships with physicians, clinicians, and administrative leaders.
