Harbor Health looking for a team-oriented Advanced Practice Registered Nurse/Physician’s Assistant to become a member of our team. Advanced Practice Registered Nurse/Physician’s Assistant will be a clinical leader providing effective, efficient, and appropriate services to Harbor Health clients in a primary clinic. The Advanced Practice RN/PA will demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills, for members; will demonstrate critical thinking in the clinical decision-making process; will work collaboratively with the clinical practice team to meet the needs of clients, supporting the delivery of policy and procedures, and providing leadership as required.
Our Advanced Practice Registered Nurse/Physician’s Assistant will be responsible for:
- Interviewing members to obtain history, performing physical examinations, ordering lab and other tests, prescribing medications, and treatments.
- Providing continuity in managed care for clients with pre-existing long-term problems.
- Making referrals for secondary and tertiary care as needed and in collaboration with care team.
- Performing medical procedures according to privileges issued.
- Providing health maintenance visits, evaluating for immunizations, and providing anticipatory guidance and referrals; performing contract and special physical examinations for employment, etc.; performs STD (sexually transmitted disease) and family planning screening and education.
- Documenting and coding all client contact accurately and legibly in the medical record.
- Performing as an active member of the medical team.
- Participating and managing care team meetings
- May be asked to perform other duties that align with the organization’s mission, including but not limited to participation on clinical committees, providing education, Quality Review and/or other activities.
Successful Advanced Practice Registered Nurse/Physician’s Assistants will have:
- Graduate of an accredited Physician Assistant Program and certified by the National Commission on Certification of Physician Assistants (NCCPA) or Nurse Practitioner Program and certified by the American Nurses Credentialing Center or the American Academy of Nurse Practitioners.
- Current licensure as a PA/NP to practice in the State of Texas by the State Boards.
- Licensed and certified to practice in Family Medicine.
- Current DEA and CPR certification required.
- Valid Texas Driver’s License.
- Must be able to meet and maintain current organizational credentialing and privileging requirements.
- Comfortable as solo provider in a clinic team setting
- Computer literate with ability to enter information in the E.H.R. system and compile reports or data as requested
- Ability to read, write, and speak English
- Skill in providing excellent customer service and support; organizing and prioritizing workload and meeting deadlines; and excellent written and verbal communication.
- Ability to interact effectively and professionally with persons from diverse cultural, socioeconomic, education, racial, ethnic, and professional backgrounds.
- Ability to work effectively with managers, co-workers, members of the public and professional groups.
- Ability to communicate effectively, clearly, concisely with others (internal and external customers, both verbally and in writing), consistently demonstrate positive/proactive customer service attitude.
- Consistently maintain ethical behaviors exemplary of quality public service and fair standards, inclusively, among all employees and members of the public.
- Ability to work as an effective team member; function independently, exercise sound judgment and initiative; be flexible to shift priorities; maintain confidentiality; establish and maintain effective interpersonal work relationships, and effectively assist other providers.
Additional Skills & Experiences Preferred include:
- 2 years’ experience as APRN/PA in a primary care setting.
- Bilingual English/Spanish
Physical Requirements of the role include:
- Working irregular hours
