NP/PA- Senior Care Services

ThedaCare, Wisconsin's largest non-profit healthcare system is seeking an NP/PA to join our team Senior Care Services team. Responsibilities include caring for those in the (PALTC) setting and building relationships with these patients.  

Highlights:

• 1.0 FTE
• Hours: Monday-Friday, between 7a-5p
• Locations: Fox Valley, Shawano, Berlin, and Waupaca (Cross coverage to help cover PTO)
• Average patient census: 8-12
• Call: 1:23 (phone call only)
• Great Group of Support from Team which includes: 5 physicians and 8 APC's
• Will work independently 

Looking for an experienced APC or will consider a new grad with a background in Long-term care.

• ThedaCare Medical Center-Berlin, 25 Bed Critical Access
• ThedaCare Medical Center-Wild Rose, 25 Bed Critical Access
