Palliative Care Nurse Practitioner or Physician Assistant

Ideal candidates will be willing to work with multi-disciplinary teams including inpatient-rounding teams, hospice and skilled nursing facility teams, program development teams, outpatient care teams and support groups. • Ideal candidate will possess excellent interpersonal skills with diverse populations including patients, medical staff and faculty, residents, nursing staff, social workers, case managers, chaplains and other provider personnel
