Most importantly, this provider will help develop and implement a Chronic Care Management (CCM) program, working with chronically ill or “high-risk” patients with the goal of enhancing patient health and the ability for self-management and shared decision-making. Other program goals will include reduction in emergency room utilization and hospital readmissions. This provider will be part of a CCM team that will assess and address patient’s unmet health and social needs, assist patient with development of treatment goals, and help patient negotiate cognitive, physical, social, and economic barriers to care.
