Mercy Health-Lourdes Hospital is seeking a Palliative Care Nurse Practitioner for a full-time employment opportunity to provide palliative services in a non-acute setting. The candidate will continue a community palliative care program with the goal of reduction of ED visits, reduction of acute care hospitalization, reduction in 30-day acute care readmission, reduction of health care cost and the increase of patient/family experience of care ratings. Responsible for seeing 6-8 patients daily depending on travel time.
