Essential Duties and ResponsibilitiesMedical Record Review and Analysis- Conduct thorough reviews of patient medical records to identify gaps in care, potential risks, and areas for improvement
- Evaluate treatment plans for alignment with best practices and evidence-based guidelines
- Document findings and actionable recommendations clearly and concisely
Improvement Plan Development- Develop and implement quality improvement initiatives to enhance patient care and operational processes using StayPut data
- Collaborate with regional and market teams to identify and address areas for improvement
- Focus on chronic condition management within patients' current care environments
Clinical Decision-Making Support- Provide expert guidance to caregivers and staff to prevent avoidable hospital admissions and ED visits
- Address barriers to effective care delivery through targeted interventions
Education and Training- Offer clinical education and quality improvement coaching to clinicians in the Call Solution team
- Educate nursing and care staff on best practices and early intervention techniques
- Provide ongoing onboarding and education for clinical team members
Interdisciplinary Collaboration- Work closely with physicians, nurses, social workers, and other team members to ensure comprehensive patient care
- Participate in interdisciplinary team meetings to review patient progress, care strategies, and program goals
Program Development and Evaluation- Contribute to refining StayPut protocols and workflows to enhance patient outcomes
- Develop and maintain StayPut resources to promote the program within partner facilities
- Analyze program metrics to evaluate effectiveness and identify areas for improvement
Compliance and Quality Assurance- Ensure adherence to regulatory and clinical standards
- Participate in audits and quality assurance initiatives as needed
Qualifications and Experience- Education: Master's Degree in nursing or Master's Degree in physician assistant studies
- Licensure: Active nurse practitioner (NP) or physician assistant (PA) license
- Experience: Minimum 3 years' experience as NP or PA in post-acute care or quality improvement initiatives
Skills- Strong knowledge of quality improvement methodologies and tools
- Excellent communication and interpersonal skills
- Ability to work collaboratively with multidisciplinary teams
- Strong analytical and problem-solving abilities
- Proficiency in data analysis and reporting
