HarmonyCares is seeking skilled and compassionate Nurse Practitioners & Physician Assistants to join our team in Gary and Crown Point, Indiana!

Position Highlights:

  • Provide coordinated primary care to seniors in their homes.
  • Manage a smaller patient panel (8-10 patients per day) for more personalized care.
  • Spend up to 45 minutes per visit, allowing for meaningful patient relationships.
  • Work with a supportive team, maintaining a balance of clinical autonomy and collaboration.
  • Be accompanied by a Medical Assistant, with transportation provided for patient visits.
  • Monday-Friday schedule – No weekends!

Position Requirements:

  • Valid Nurse Practitioner or Physician Assistant license in the state of Indiana.
