Position Overview: We are seeking a dedicated and detail-oriented Advanced Practice Provider (APP) to join our team. The APP will collaborate closely with Nephrologists and the Clinic Manager to deliver exceptional patient care both in the hospital and clinic settings.
Work Schedule:
Rotation Structure: The APP will follow a consistent rotation schedule:
1 week in the hospital (Monday – Sunday) followed by 4 weeks in the clinic.
The week following the hospital rotation will consist of 3.5 clinic days, with a half-day dedicated to administrative duties to compensate for the previous weekend’s work.
Volume Expectations:
Hospital Shifts: The APP will collaborate with a Nephrologist to round on inpatients, assisting with care planning and treatment strategies.
Clinic Shifts: APPs are expected to see approximately 11-12 patients during a full clinic day.
Call Schedule:
There are currently no on-call requirements for APPs in this role.
Daily Responsibilities:
Hospital Duties:
Meet with the assigned Nephrologist to strategize daily patient care coverage.
Review patient labs, imaging, and other relevant data to develop or adjust inpatient care plans.
Clinic Duties:
Conduct patient appointments based on a variety of schedules, with intervals ranging from weekly to annually.
Utilize designated time blocks to review patient charts and prepare for upcoming appointments.
