Schuyler Hospital/ Seneca View Skilled Nursing Facility Nurse Practitioner Position

Responsibilities

• Completion of new admission orders and medication reconciliation. 
• Perform acute patient evaluations and interventions as needed.  This may include referral and follow-up care.
• Assist with regulatory-mandated patient/resident assessments in compliance with federal and state guidelines.
• Complete comprehensive discharge assessments and discharge orders (medication, labs, studies, referral, etc.).
• Maintain accurate documentation and completion of progress notes according to hospital policy.
• Coordinate with other providers and support staff when needed.
