Nurse Practitioner - Tuolumne, CA

Job Summary:
The Nurse Practitioner (NP) is a licensed independent practitioner responsible for meeting, assessing, and diagnosing patients in our clinic while providing healthcare services that meet State and National standards. The provider will be accountable for health promotion, prescribing appropriate medications, health regimens, and lifestyle adjustments to contribute to patient recovery, prevent future health issues, and promote overall wellness. The Nurse Practitioner will deliver high-quality, patient-centered care, ensuring compliance with all regulatory standards while working in a collaborative and multidisciplinary healthcare environment.

Responsibilities:

• Patient Assessment & Care

  • Independently perform comprehensive history and physical examinations for perioperative and primary care patients.
  • Assess both normal and abnormal findings in patient health.
  • Evaluate and treat acute injuries and illnesses.
  • Provide preventive healthcare services and monitor intervention effectiveness.
  • Order and interpret diagnostic tests, prescribe medications, and implement treatment plans in compliance with federal and state regulations and Indian Health Service (IHS) guidelines.

• Coordination & Follow-Up

  • Coordinate follow-up services, including lab work, imaging, and specialist referrals.
  • Schedule appointments through the Medical Receptionist and ensure seamless transitions between healthcare settings.
  • Manage admissions, transfers, and discharges while collaborating with a multidisciplinary team.
  • Consult with the Medical Director or Provider Practice Lead (PPL) when cases exceed professional scope.

• Health Education & Community Engagement

  • Provide patient and family education on health promotion, risk management, and disease prevention.
  • Work with community organizations to connect individuals with necessary resources.
  • Contribute to staff development by acting as a resource in clinical areas of expertise.

• Regulatory & Quality Assurance

  • Engage in continuing education and uphold licensure requirements.
  • Participate in quality assurance initiatives, ensuring compliance with QA guidelines.
  • Update medical forms, clinic protocols, and assist with inventory management.
  • Report public health concerns (e.g., epidemic outbreaks, accidents) to the Department of Health Services and local authorities within the required timeframes.

• Other Duties

  • Participate in the Quality Assurance Committee.
  • Ensure compliance with all regulatory standards for patient care and safety.
  • Coordinate with the Department of Health Services during public health emergencies.

Compensation:

• Salary: $136,105.28 – $163,105.28
• Sign-On Bonus: $4,000
• Relocation Assistance: $6,500

Schedule:

• Option 1: Tuesday – Friday, 7:00 AM – 6:00 PM
• Option 2: Monday – Friday, 8:00 AM – 5:00 PM

Setting:
Federally Qualified Health Center (FQHC), providing primary care services to both Native and non-Native community members.

Qualifications & Requirements:

• Doctor of Medicine or Doctor of Osteopathy from an accredited institution.
• Completion of residency program in a relevant specialty (e.g., Family Medicine, Internal Medicine).
• Board Certification or Board Eligibility in the specialty.
• Valid California medical license, DEA certification, and BLS certification.
• Ability to pass a comprehensive background investigation (must be free of felony convictions).
• Must possess a vehicle, a valid California driver’s license (or obtain one within 30 days of hire), and vehicle insurance.
• BLS and ACLS certifications required.

Physical Requirements:

• Ability to walk, stand, and sit for prolonged periods.
• Capability to climb stepstools, stoop, kneel, crouch, and reach.
• Lift up to 30 pounds and raise objects overhead.
• Manual dexterity for operating computers, phones, and small office machines.
• Ability to communicate effectively and efficiently in both verbal and written forms.
• Repetitive motion of wrists, hands, and fingers.
