Geriatrics/Skilled Nursing Facility Program - Nurse Practitioner (NP) with SSM Health in Southern WI/ Northern IL

At SSM Health, we are committed to delivering exceptional healthcare services. Our Monroe Market team is growing and seeking a dedicated Geriatric Nurse Practitioner to join our nursing home team. In this unique role, you will provide patient care traveling to skilled nursing facilities in the greater Monroe area (southern Wisconsin and Northen Illinois).

Position Highlights:

• Employed position with SSM

• Schedule: Monday-Friday, 8a-5p.
• NO nights/weekends/holidays or call!

Minimum Qualifications:

• AGNP highly preferred; open to considering an FNP as well
• Geriatric NP experience highly preferred; also open to new grad NPs who have had geriatric/post-acute & long-term care training/exposure (Geriatric NP exp., Geriatric NP clinical rotations, Geriatric RN experience)
• Ability to obtain NP licensure in WI and IL

Salary Range:

$101,732.80 - $152,568.00

At SSM Health Monroe Hospital, a state-of-the-art, 58-bed facility built in 2012, our goal is to serve the community and play an active role in it. An added level of intensive and direct patient care offered at SSM Health Monroe Hospital is e-ICU. By adding e-ICU capabilities, bedside nurses and doctors are supported by and can collaborate with experts at UW Health to enhance patient outcomes and help eliminate future health problems. Other departments housed in our hospital building include the surgical services, cardiology, physical therapy, medical imaging, and emergency services.
