Practice Overview:

• EMR Platform – EPIC

• Inpatient care only

• Palliative Care team currently consists of three (3) Physicians, two (2) APPs, a two (2) Social Workers and a Chaplain

• Group call coverage shared amongst group members – telephone only

• Schedule is Monday – Friday, 8a-5p

• Will participate in initial consults, develop and implement goals of care

• Exceptional employed Hospitalist & Critical Care team, consisting of over 125 providers, offers 24/7 support

• Opportunities to work with our Hospice and Palliative Care Fellows

Do you have what it takes?

• PA or ACNP/AGACNP

• If not certified in Hospice/Palliative Care, selected candidate must be willing/able to obtain certification within 1 year of hire through ACHPN

• Functions well in a team environment, contributing to and supporting organizational goals and initiatives

• Two years of clinical experience as an APP preferred

• Oncology, Acute Care or Palliative Medicine experience preferred

• Knowledge of palliative care, acute care, hospice, pain management, advanced directives and end of life decision making
