Inpatient Diabetes Management APP

The Inpatient Diabetes APP serves at the site lead for inpatient diabetes operations, functioning within the hospital environment to provide direct and indirect patient care. 

Role Specifics:

• Serves as facility lead for inpatient diabetes operations
• Dedicated consultation service for education and management of patients with diabetes 
• Provides direct clinical care for patients with diabetes according to established protocols and collaborates with interdisciplinary team
• Provides education to patients and families for self-management of acute or chronic illnesses, tailoring instructions to patients' individual circumstances
• Delegates diabetes education to other members of the diabetes care team, as appropriate
• Collaborates with interdisciplinary care team for discharge planning for patients with diabetes

Qualifications:

• Current Licensed Physician Assistant or Advanced Practice Nurse Practitioner
• Current NCCPA certificate as a Physician Assistant or AACC certificate as an Advanced Practice Nurse
• Physician Assistant or Advanced Practice Nurse Practitioner controlled substance licensure
