Primary Care - Advanced Practitioner (FNP or PA) - Fond du Lac, WI

This part-time opportunity focuses on providing a patient-centered approach to both primary and acute care for Grande Cheese associates and their families. New and recent graduates are welcome to apply.

  • Flexibility in business hours - clinic can be open between 7a-6p Monday-Friday
  • Position will a combination of Mondays, Wednesdays, and Fridays

Key Responsibilities:

• Provide independent patient care
• Conduct comprehensive primary and acute care visits
• Perform pre-employment and work-related physicals
• Develop and participate in population health programs
• Utilize EPIC Electronic Medical Record system

Qualifications:

• Advanced practitioner certification - PA or FNP
• Ability to obtain license in Wisconsin
• Experience in primary and acute care preferred 
• Strong patient communication skills
• Commitment to personalized, high-quality healthcare
