Position Highlights:

  • 1.0 FTE/ 40 hours/5 days a week + (1 weekend a month)
  • Hackensack Hospital (Internal Medicine)
  • Average of 15-20 patients per day
  • NO CALL
  • Works with Hospitalists Physician supervision for admissions
  • Physician-led, Patient-centered team-based care environment
  • Partner with other medical professionals while practicing with autonomy.
  • Practice at the peak of your license providing evidence-based medicine.
  • Patient Assessments, Formulating Care Plans, Finding Gaps in Care Delivery, Communicating and Teaching patients about personal Care Management
  • Educate and empower your patients to take ownership of their health.
  • State of the art Epic EMR system

 

Primary Responsibilities:

  • Direct and manage a complete continuum of high-quality care within the care team. 
  • Treat chronic diseases and disorders 
  • Reviews patient file/record, including allergies, problems, medications, and immunization status. 
  • Elicits and records information about patient’s medical history. 
  • Orders or executes various tests, analyses, and diagnostic images to provide information on patient’s condition. 
  • Analyzes reports and findings of tests and examination and diagnoses condition of patient.

Required Qualifications:

 

  • Master’s degree as Nurse Practitioner or Physician Assistant from an accredited program.
  • Certified as a Nurse Practitioner through the ANCC/AANP, or as a Physician Assistant through the NCCPA.
  • Active and unrestricted Nurse Practitioner (Advanced Practice Nurse, ANP) or Physician Assistant license in the State of New Jersey.
  • Active and unrestricted CDS permit (or ability to obtain prior to start).
  • Active and unrestricted DEA License (or ability to obtain prior to start).
  • Must be CPR certified (or willing to obtain prior to start).
  • Prior relevant post-graduate clinical experience highly preferred.

The salary range for this role is $89,800 to $176,700 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
