Job Description Summary
We are a rapidly expanding team of Hospitalist physicians and advanced practice providers committed to clinical quality improvement and ensuring patients receive the highest quality care at an affordable cost.
The Hospitalist Advanced Practice Provider (APP) will play a critical role in the management of hospitalized patients, with a primary focus on those on general medicine floors. The APP will work collaboratively with attending physicians, tele-critical care, nursing staff, and other healthcare professionals to provide comprehensive care to adult patients, ensuring high-quality, patient-centered outcomes.

Key Responsibilities:
- Manage acute and chronic medical conditions, including initiating appropriate diagnostic and therapeutic interventions while practicing efficient and judicious use of hospital and healthcare resources.
- Conduct comprehensive assessments, including history and physical examinations, review of labs, vitals and medication reconciliations.
- Develop, implement, and evaluate treatment plans with collaborating physicians.
- Attend Multidisciplinary Rounds; work with multidisciplinary teams, including consulting physicians, nurses, case management, respiratory therapists, and pharmacists, to ensure coordinated patient care, provide input on patient management, care plans and discharge planning.
- Promote effective communication among patients, families, and healthcare teams, ensuring that patients and their families are well-informed and actively involved in care decisions. Offer education and support to patients, families, and healthcare staff on disease processes, treatment plans, and care transitions.
- Maintain accurate and timely documentation of patient encounters, including history and physical, progress notes and discharge summaries in the electronic health record (EHR) system. Ensure that records are detailed, complete and comply with hospital policies, regulatory requirements, and best practice standards.
- Engage in quality improvement initiatives within the hospitalist program and the hospital. Participate in hospital committees, task forces, and meetings as required.
- Using quality and process improvement techniques and practices to make the hospital a safer place and improve patient outcomes. Stay abreast of current trends in healthcare practice and participate in continuing medical education to ensure optimal clinical knowledge and effective patient care skill. Knowledge of core competencies of clinical conditions.
