Family Medicine Nurse Practitioner Primary Care

The Nurse Practitioner will be responsible for taking history and physical for both new and established patients of all ages, order appropriate testing, order appropriate treatment, interpret labs and other diagnostic results, document in electronic medical chart, prescribe DME, prescribe medications and refills, decide on a plan of treatment with physician, and other duties as required. Must be able to multi-task, take charge and provide direction to other clinical staff. 

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Within the scope of a typical family medicine practice treats acute illnesses, manages chronic illnesses and provides comprehensive, patient-centered and science based preventive care services and health education.
  • Treats acute and urgent conditions within scope of license and credentialing.
  • Consults with physician supervisors regularly regarding patient care and reviews care plans as needed with consulting physician.
  • Participates in organization sponsored recognition, performance improvement and practice transformation activities.
  • Assists in the development and training of clinical guidelines and procedures.
  • Performs health assessments and preventative health measures within prescribed guidelines and physician instructions. 
  • Does clinical procedures within the scope of training and experience, including but not limited to contraceptive implants, IUDs, skin biopsies, I and D, suturing, and joint injections.
  • Provides phone consultation with patients, responds to patient inquiries, and reviews test results with patients.
  • Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs. Discusses case with physician. Formulates and documents care plan. 
  • Proactively manages all electronic messages from the electronic health record and patient portal and responds to these messages within prescribed timeframes as set by the medical director.
  • Participates in telephone triage in the absence of nursing staff.
  • Prescribes medication or other forms of treatment. 
  • Provides health education and support to patients and their families.
  • Participates actively and contributes to organizational projects, meetings and activities as needed.
  • Participates in oversight and training of medical assistants and other support staff.
  • May be required to work in multiple sites of service in support of organizational needs.

POSITION REQUIREMENTS

 Education

  • Master’s Degree in Nursing required
  • Must have current certification via American Nurses Credentialing Center (AANP) or American Academy of Nurse Practitioners (ANCC). Current and without restriction RN and NP license required

Qualifications/Experience

  • Minimum of one year experience as an NP in clinical practice
  • Experience as RN or NP in primary or urgent care setting
  • Prescribing authority/DEA licensure in good standing 
  • Current Basic Life Support (BLS) for Health Care Providers from the American Heart Association
  • Current Advanced Cardiac Life Support (ACLS) for Health Care Providers from the American Heart Association

Special Skills/Knowledge

  • Bilingual skills preferred (Spanish)

  • Demonstrated ability to interact effectively with people at all levels of the organization
  • Ability to promote and maintain patient health by providing medical services in collaboration with their supervising physician.
  • Ability to obtain detailed and accurate medical histories and performs appropriate physical examinations on patients across the lifespan.
  • Ability to develop patient-centered care plans which incorporate counseling on healthy lifestyle choices, preventative medical care and psychosocial stressors, as well as coordination of referrals to specialty and behavioral health services.
  • Ability to perform and/or interpret common laboratory, radiological, and other routine diagnostic procedures- as needed.
  • Ability to perform medical procedures/exams such as pap smears, wound care, suturing, joint injections, administration of immunizations or injections, and infection management based on current privileges.
  • Ability to work collaboratively on a physician team to care for medically complex patients.
  • Ability to maintain accurate and up to date medical records and billing.
  • Ability to maintain safe and clean working environment by complying with clinic and health care system defined policies, procedures and regulations.
  • Demonstrated ability to protect patients and employees by adhering to infection-control policies and protocols.
  • Ability to demonstrate sensitivity and awareness regarding the emotional, cultural and socioeconomic aspects of the patient, the patient’s condition, and the patient’s family and social area.
  • Ability to maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.

Physician specialties include Family Medicine, Internal Medicine, Endocrinology, Psychiatry, Psychology, Urology, Cardiology, Podiatry, Pediatrics, Infectious Disease, Vascular Surgery, Neurology, Nephrology, OB/Gyn, and Pulmonology. MLKCH opened in July 2015 as a state of the art facility with all new equipment and technology. MLKCH has 131 licensed beds, including 93 medical/surgical beds, 20 intensive care beds and 18 obstetrical beds.
