Position Summary:

Join our dedicated Care Team at Sage Health, where you will work alongside a Physician, Care Coordinator, Medical Assistant, and Nurse Care Manager to provide comprehensive and compassionate care to our patients. As a Nurse Practitioner, you will play a vital role in delivering longitudinal care, fostering meaningful relationships with patients, and helping them navigate their care plans.

In this collaborative environment, you will be responsible for a select panel of patients, ensuring high-quality care both in our centers and in patients' homes. Your efforts will focus on annual health assessments, post-discharge follow-ups, and the coordination of care, all aimed at enhancing the well-being of our community.

Primary Responsibilities:

• Evaluate and treat patients in accordance with established standards of care.
• Coordinate care with other healthcare providers, specialists, and care facilities.
• Conduct comprehensive in-home assessments for designated patients, aiding the primary care team in managing chronic diseases effectively.
• Perform physical examinations, medication reviews, vital sign checks, lab draws, wound care, and depression screenings.
• Communicate assessment findings to the Primary Care Physician (PCP) to address potential gaps in care.
• Collaborate with Medical Assistants to maximize time for direct patient interaction.
• Build strong relationships with patients, particularly focusing on the aging and elderly population, to provide passionate service.
• Educate patients on disease processes, medication adherence, and health compliance.
• Adhere to all HIPAA regulations and maintain the confidentiality of protected health information.

Qualifications:

• Current and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license or eligibility.
• Experience with risk adjustments and home assessments.
• Knowledge of HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) coding, and Health Risk Assessments (HRAs).
• Strong commitment to teamwork and collaboration with skilled colleagues.
• Completion of an accredited Nurse Practitioner Program or Physician Assistant Program.
• Current DEA certification required prior to the start date.
• BLS certification.
• A minimum of two years of experience as a Nurse Practitioner or Physician Assistant.
• At least two years of experience in Primary Care or Adult/Geriatrics.
