Primary Function/General Purpose of Position
The Heart Failure Extensivist will provide virtual care for patients with heart failure with a primary focus on optimization of guideline-directed medical therapy (GDMT). When a primary care provider (PCP) is managing a patient with heart failure, optimizing GDMT may not be feasible within the constraints of a conventional practice. The extensivist will function to support the PCP. Upon referral by the PCP, the extensivist will order guideline-concordant medications and lab testing while the patient is enrolled in a remote patient monitoring program. Once the maximum tolerated medical regimen is achieved the patient will graduate and care will be transitioned back to the PCP. Patients may also be referred by cardiologists or conventional CHF clinics, particularly for patients in whom travel to an office setting is difficult or undesirable. The extensivist will also provide virtual care for patients transitioning from hospital to home after an admission for heart failure and assist in the virtual care of patients with acute congestion being managed within primary care. The extensivist will function under the supervision of a cardiologist.
Essential Job Functions
Optimize guideline-directed medical therapy for heart failure.
Order appropriate lab testing and manage the results in a timely manner.
Prescribe oral, IV, or SC medication as indicated and where feasible.
Receive and act on communications from the remote patient monitoring team regarding enrolled patients.
Educate patients on heart failure and self-monitoring and management.
Manage any heart failure or treatment-related problems that may arise while patient is enrolled in the program.
Refer patients to cardiac rehab where available and in accordance with AHA/ACC guidelines
Attend administrative meetings related to the extensivist program.
Provide one-time virtual encounters for ambulatory patients with acute congestion detected by remote patient monitoring if managing provider is unavailable.
Contribute to PCP and CHF clinic APC and care manager education on heart failure management.
Contribute to the success in heart failure-related system KPIs.
